Case Management Service for Grandcarers
Uplyft is pleased to announce the launch of a new case management service for Grandcarers proudly funded by the Stan Perron Charitable Foundation.
This service is provided by experienced social workers and family support workers who will assess the needs of Grandcarer families and help them access vital support services.
Overview
- The service aims to enhance the safety, health, and wellbeing of grandchildren in your care.
- You will receive personalised, one-on-one support.
- You will have access to practical resources and services tailored to your family’s needs.
Eligibility
Available to new and current Grandcarers who are eligible for the Uplyft Grandcarer Support Scheme (GSS).
Duration
- This service offers short-term intensive support to build your capacity and support networks.
- The service is usually offered over a period of 6 weeks to 4 months depending on family circumstances.
